If you wish to register as a patient please complete the registration forms (available online and at the reception) and bring in your medical card, I.D or passport and one proof of address (bank statement or utility bill) 

It is our requirement that all new patients attend the surgery at their earliest convenience for a new patient medical, which is usually undertaken by the practice nurse. ( at the moment face to face appointments are offered by the GPs only)

Although you register with a specific doctor you are free to consult any of the doctors.  It is obviously an advantage to see the same doctor regularly for any one complaint so that progress can be assessed and continuity of treatment maintained.

We would appreciate notification of any change of name, address, telephone number or marital status as soon as possible so that our records can be kept up to date.

Medical treatment is available from the date of registration. Please contact reception for further information.
